News Release: INTEGRATED COMMUNITY CARE CENTER RIBBON CUTTING CEREMONY

LOS ANGELES – VA Greater Los Angeles Healthcare System (VAGLAHS) will be hosting a Ribbon Cutting Ceremony on 29 Aug. from 10 a.m. to Noon for the new Integrated Community Care Center (ICCC) Veteran homeless program located on the VA West Los Angeles Medical Center Campus, Building 402.

The ICCC streamlines care provided to homeless Veterans by providing same-day access to housing, primary care, mental health, and substance use treatment delivered by trans-disciplinary care teams and Enhanced Housing First teams.

In addition, staff from programs such as Department of Housing and Urban Development- VA Supportive Housing (HUD-VASH), Grant Per Diem, Homeless Veteran Supportive Employment Program, Outreach, and Community Residential Care offers services to homeless Veterans in the building.

“The ICCC is a great example of how VA continues to provide our Veterans with Personalized, Proactive, Patient-driven Healthcare,” said Donna Beiter, Director of VAGLAHS. “Veterans who are homeless or at risk of becoming homeless can receive assistance and support in a warm and inviting environment.”

The program has dedicated space for representatives from Veterans Benefits Administration, Social Security Administration, Los Angeles County Department of Mental Health, Pro Bono Legal Services through a Medical Legal Partnership, and the Supportive Services for Veteran Families program.

“The ICCC embraces person centered care; in which the individual has many pathways out of homelessness using a ‘one-stop-shop’ approach,” said Michelle Wildy, Chief of Community Care. “This user friendly approach is what our Veterans really need.”
