Getting involved in post-acute care has become an increasingly popular strategy for many hospitals and health systems recently. This has largely been driven by the Patient Protection and Affordable Care Act (PPACA).
A goal of the PPACA, aka healthcare reform, is to make the entire care team collectively accountable for patient outcomes across the complete continuum of care, from wellness and outpatient care through inpatient and post-acute care. As bundled payments hinging on patient outcomes increasingly replace the traditional fee-for-service model, hospitals have a powerful new incentive to stay involved throughout the process.
