News Release: Onicx Healthcare Real Estate Expands into Tennessee with Acquisition of Nashville-Area Medical Outpatient Building

Onicx Healthcare Real Estate Fund acquires Crossroads Medical Pavilion, a 100%-leased, physician-anchored 33,665-square-foot medical outpatient building in White House, Tenn.

WHITE HOUSE, Tenn. – August 5, 2026 – Onicx Healthcare Real Estate (“OHRE”), the healthcare investment and development platform of Tampa-based Onicx Group, is pleased to announce the acquisition of Crossroads Medical Pavilion, a one-story, 33,665-square-foot medical outpatient building in White House, Tennessee, approximately 25 miles north of downtown Nashville. The acquisition was made through the Onicx Healthcare Real Estate Fund and is OHRE’s first in Tennessee, extending the platform into one of the Southeast’s fastest-growing metropolitan areas. It follows Fund acquisition and development investments in Indiana and Florida.

Located at 491 N. Sage Road, Crossroads Medical Pavilion was completed in 2007 and purpose-built for Crossroads Medical Group — now operating as Bookmark Medical — the multi-specialty and primary care practice that has anchored the building since it opened. The practice occupies 16,882 square feet, or 50% of the building, and extended its lease by an additional seven years in connection with the transaction, remaining the property’s anchor tenant under OHRE ownership.

The property is 100% leased to a synergistic roster of regional and national healthcare providers, including Ascension Saint Thomas, TriStar Skyline (HCA), Access Dermatology Clinic, Star Physical Therapy and White House Pharmacy — a tenant mix that pairs health-system credit with entrenched independent physician practices. The building has benefited from recent capital investment, including a full roof replacement in 2021 and HVAC system upgrades.

“Tennessee has been a target market for us for some time, and Crossroads Medical Pavilion is exactly the kind of asset we want to own — a fully leased, physician-anchored outpatient building in a high-growth suburban community with real health-system presence,” said Jon Sajeski, President of OHRE. “As care continues to move out of the acute-care setting and closer to where patients live, well-located community medical buildings like this one become increasingly essential infrastructure. This acquisition establishes our platform in the Nashville metro and gives us a foundation to grow from.”

“What made this acquisition compelling was the combination of a long-standing clinical anchor and the opportunity to strengthen the property’s lease profile,” added Ilya Hvostikov, Vice President of Acquisitions at OHRE. “During diligence, we worked with the seller to bring the building to full occupancy and secure a seven-year extension from the anchor practice. That execution increased our conviction in an asset that has served the White House community since 2007.”

White House straddles Sumner and Robertson counties along the Interstate 65 corridor and is among Middle Tennessee’s fastest-growing suburbs, with a population of approximately 18,700 — an increase of more than 40% since the 2020 census and current annual growth of roughly 4.7%. Average household income within five miles of the property is approximately $123,300.

OHRE plans to build on the property’s recent capital investment with a program that includes modernizing the building’s common areas, enhancing property signage and expanding the parking lot to better accommodate patients, physicians and staff.

Andrew Larwood and Allen Inman of Atlanta-based Capital Real Estate Group represented the seller in the transaction. Tyler Morss of Cushman & Wakefield’s Healthcare Capital Markets Team led the assignment, arranging $5.34 million in acquisition financing on behalf of OHRE, provided by Northbrook Bank & Trust Co., a subsidiary of Wintrust Financial Corporation.
